Chapter 1

The Blood on the Cobblestones

The autumn rain fell over Palermo like a sodden shroud, drumming against the clay-tile roofs of the Il Capo district and turning the ancient limestone gutters into rushing torrents of black water. In the narrow, labyrinthine alley of Vicolo delle Vergini, shadows pooled beneath crumbling baroque archways, smelling of wet volcanic stone, rotting figs, and diesel exhaust. It was two in the morning—the dead hour when the street markets were shuttered with padlocked iron chains and only ghosts, stray dogs, and killers walked the cobblestones.

Behind the frosted, reinforced-glass window of a nondescript ground-floor storefront, Dr. Valentina Rossi stood over an enamel washbasin, scrubbing the sting of iodine from her knuckles.

The brass plaque beside the door did not bear her name, nor did it advertise medical practice. To the neighborhood, it was an unmarked herbal apothecary; to the desperate, the undocumented, and the hunted who dared not set foot inside the surveillance-choked emergency rooms of the Civico hospital, it was the only sanctuary in western Sicily. For six years, Valentina had operated in the dark, suturing stab wounds, delivering babies under humming kerosene lanterns, and treating gunshot fractures without filing police reports.

She was twenty-six years old, slender and graceful, with thick chestnut-auburn hair pinned in an efficient surgical knot at the nape of her neck. Her eyes were a rare, striking hazel-gold—the color of aged amber held against candlelight—reflecting a chilling, unyielding calm that had been forged in tragedy. Eight years ago, her father, Anti-Mafia Investigating Magistrate Antonio Rossi, had been obliterated by fifty kilograms of military-grade Semtex planted beneath his Alfa Romeo outside their villa in Mondello. The blast had ripped her adolescence to shreds, sent her younger sister Gianna into hiding in a cloistered convent, and taught Valentina that justice in Sicily did not wear robes; it was bought, buried, and paid for in blood.

The violent thud against the clinic’s heavy oak door rattled the surgical instruments in their stainless-steel trays.
